Now Accepting New Patients
The Ottawa Nurse Practitioner-Led Clinic is currently accepting new patient applications. We provide primary health care services to individuals who live in or meaningfully reside within the K1L or K1K postal code areas, including those who may be precariously housed or experiencing homelessness in the Vanier/Overbrook community.
To be eligible for registration with our clinic, two criteria must be met:
You must live in the K1K or K1L postal code areas.
You must not currently have a primary care provider (family doctor or nurse practitioner).
If you live outside K1K or K1L and already have a primary care provider, we kindly encourage you to register with Health Care Connect at 1-800-445-1822 or through their website: https://www.ontario.ca/healthcareconnect.
If you live within K1K or K1L and do not currently have a primary care provider, you may proceed with registration at ONPLC. To place yourself on our intake priority list, please complete our secure registration form here:

We are currently experiencing a high volume of new patient applications and are working carefully to review and respond to each one.
Our team is committed to welcoming new patients while ensuring continued access and quality care for those already enrolled. We kindly ask for your patience and understanding as we work through the applications.
Please note: This clinic is intended for individuals who do not currently have access to primary care.
If you choose to become a patient at our clinic, you will be required to discontinue care with any other primary care provider.

Booking an Appointment
The Ottawa Nurse Practitioner-Led clinic strives to offer same day or next day appointments for patients who have illnesses that arise suddenly. If you are sick and need to see a Nurse Practitioner today or tomorrow, please call the office.
Prescription renewals and any other non-urgent issues should be by appointment only and arranged in advance. Please book an appointment for renewals one month before your medications run out.
Please bring your health card and medications to every visit. Inform reception of any changes of name, address, phone numbers, pharmacy, and emergency contact.
If calling to book your appointment, please tell the receptionist your reason for the visit. This allows for the appropriate amount of time to be booked.
Cancellations
Please call us 48 hours in advance or as soon as possible to cancel your appointment.
Post-Hospital Follow-Up
If you have recently been discharged from the hospital, please call us when you get home to schedule a follow-up visit with your Nurse Practitioner or a member of the team.
