Patient Attachment Through Health Care Connect and Community Referrals
The Ottawa Nurse Practitioner-Led Clinic has experienced very high demand for primary care services since opening. In order to maintain safe, high-quality care for patients already registered with the clinic, new patient self-registration is currently closed.
At this time, the Ottawa NPLC is attaching new patients through the Health Care Connect waitlist. Individuals who do not currently have a primary care provider are encouraged to register with Health Care Connect.
The clinic will also continue to accept referrals from community agencies and partner organizations to support individuals with the greatest needs.
The Ottawa NPLC remains committed to improving access to primary care in our community and is working closely with health system partners and funders to expand service capacity over time. Updates will be posted on this page if the patient attachment process changes.
The Ottawa NPLC will continue working with Ontario Health, Health Care Connect, and community partners to attach patients as capacity allows, with a focus on individuals with the greatest needs in our community.

Booking an Appointment
The Ottawa Nurse Practitioner-Led clinic strives to offer same day or next day appointments for patients who have illnesses that arise suddenly. If you are sick and need to see a Nurse Practitioner today or tomorrow, please call the office.
Prescription renewals and any other non-urgent issues should be by appointment only and arranged in advance. Please book an appointment for renewals one month before your medications run out.
Please bring your health card and medications to every visit. Inform reception of any changes of name, address, phone numbers, pharmacy, and emergency contact.
If calling to book your appointment, please tell the receptionist your reason for the visit. This allows for the appropriate amount of time to be booked.
Cancellations
Please call us 48 hours in advance or as soon as possible to cancel your appointment.
Online booking for your Primary Care Provider will be available soon. Thank you for your patience.
Post-Hospital Follow-Up
If you have recently been discharged from the hospital, please call us when you get home to schedule a follow-up visit with your Nurse Practitioner or a member of the team.
New Patients
Now Accepting New Patients
The Ottawa Nurse Practitioner-Led Clinic (NPLC) is currently accepting new patient applications. We provide primary health care services to individuals who live in or meaningfully reside within the K1L or K1K postal code areas, including those who may be in unstable housing or experiencing homelessness in the Vanier and Overbrook community.
To be eligible for registration with our clinic, two criteria must be met:
You must live in the K1L or K1K postal code areas.
You must not currently have a primary care provider (family doctor or nurse practitioner).
If you live outside K1L or K1K and already have a primary care provider, we kindly encourage you to register with Health Care Connect at 1-800-445-1822 or through their website: https://www.ontario.ca/healthcareconnect.
If you live within K1L or K1K and do not currently have a primary care provider, you may proceed with registration at Ottawa NPLC. To place yourself on our intake priority list, please complete our secure registration form here:
